The Creating Caring Communities toolkit is available to organizations that join as members. Membership gives your team access to the full set of modules, templates, and guidance needed to bring the CCC model into your own community.
Adopting a new model takes more than materials, so members are not left to work through it alone. Hospice Toronto provides a mentoring role for one to three years, supporting your organization as you recruit your coordinator, build relationships with community leaders, and adapt the core roles and activities to the strengths of your neighbourhood. The length of that support reflects how the model actually takes root, gradually and at the pace of the community itself.
