Care That Starts With Neighbours

The people best positioned to notice when someone is ill or isolated are often already living next door. Creating Caring Communities turns that idea into a program neighbourhoods can run themselves.

In partnership with local social agencies, we identify people who need support, connect them to in-home hospice palliative care, and support their caregivers. Hospice Toronto developed the model in 2008. It now runs across Canada, adapted each time to the place it serves.
